Arizona Medical Marijuana Laws, Qualifications, and ID Card Information
The State of Arizona has legalized a program for the use of medical marijuana. This program allows people with certain conditions or diseases to seek a recommendation from a certified physician; and if approved, a person can then apply for a Medical Marijuana ID Card which is issued by the state. Once the ID card is acquired, the card holder may then purchase marijuana per state guidelines for medical use. The following information contains details on qualifications and procedures for acquiring medical marijuana.
Who Qualifies for Medical Marijuana Use in Arizona?
Those persons wishing to acquire marijuana for medical purposes in Arizona must first be a legal resident of the state. Second, they must obtain an authentic written certification from a licensed doctor of medicine, doctor of osteopathic medicine, naturopathic physician, or homeopathic physician within the state of Arizona. This certification must confirm that you have been diagnosed with a debilitating medical condition and that medical marijuana would be an effective treatment for the symptoms.
Please note that you may need to bring a copy of your medical records on the day of your appointment for marijuana evaluation which should indicate a diagnosis of one of the qualifying conditions. Also, once you receive your recommendation, you must obtain your ID card by registering with the Arizona Department of Health Services.
What Diseases or Ailments Can Be Treated?
As mentioned before, many different illnesses can potentially qualify you for the use of medical marijuana. Now we’ll list them in more detail.
Medical marijuana can be acquired by patients diagnosed with glaucoma, cancer, HIV/AIDS, Hepatitis C, Lou Gehrig’s disease, Crohn’s disease (a particular type of Alzheimer’s disease) or the treatment of any of these conditions. Also, people with other chronic debilitating conditions may acquire marijuana for treatment if that condition produces cachexia or wasting syndrome, severe or chronic pain; acute or severe nausea; seizures, including those caused by epilepsy; or severe, persistent muscle spasms, which includes those caused by multiple sclerosis.
Accessing Your Medical Marijuana
Even though some patients try to claim that they have a prescription for marijuana, prescriptions are actually illegal. The federal government lists marijuana under a schedule I drug classification. This means doctors cannot prescribe it, and pharmacies cannot distribute it. This is the reason for the medical marijuana recommendations. Recommendations allow for more discretion and control over who is allowed to use medical marijuana.
Qualified patients or caregiver can possess up to 2.5 ounces of marijuana in a two-week period. The law allows certification for a certain number of dispensing locations throughout the state. These dispensaries are non-profit, and the number of certified dispensaries cannot exceed 10% of the number of pharmacies in the state. If a patient lives in excess of 25 miles away from the nearest dispensary location, they or a caregiver may be allowed to cultivate as many as 12 marijuana plants provided they are kept in a locked, enclosed area.

How to Obtain a Medical Marijuana Card
To legally be qualified to use medical marijuana in Arizona, you must complete the full ADHS online application form which is only available at http://www.azdhs.gov/prop203, and Adobe reader is required to open the file. Adobe reader is readily available online and is free to download.
Be sure to gather all of the documentation that is required to complete the application process that fits your role. Follow the appropriate instructions for adults, patients below 18 years of age, or those classified as Caregivers, whichever applies to you. Before beginning to complete the application, be sure you have an electronic copy of your photo ID, a current digital photo, an electronic copy of the certification form given to you by your physician, an electronic copy of your attestation, and a credit card to make your payment. The credit card should be MasterCard or Visa. You must verify that all data is complete and correct on each page of the application, and you must complete and submit the full application without closing or resetting your browser. There is no option to save your application and resume at a later time. If you lose the application page on your browser for any reason, you will need to restart the application process again from the beginning.
